The prescribed dose will vary with the intensity of your condition and other medical reasons. Its best medical use is achieved by adhering strictly to the guidance of a health care provider.
- Dosage: Unless otherwise indicated, the starting dosage is one tablet a day e,g, 16 mg/12.5 mg of candesartan 16mg and hydrochlorothiazide 12,5 mg. For reasons related to safety and efficacy, the doctor may increase the daily dosage up to a maximum of 32 mg/25 mg based on the effectiveness of the treatment.
- Missed dose: In case you forget to take your dose, you may take it once you remember however when it is almost time for your next dose do not take the previous one. Do not take a missed dose if the next scheduled dose is almost due, do not take two doses to make up for a missed dose.
The antihypertensive effect of Atacand HCT consists of a synergy of two actions.
- Candesartan reverses the action of angiotensin II in blood vessels, leading to vasodilation. Angiotensin II has the effect of constricting blood vessels which increases the workload of the heart hence increases blood pressure. Hence,a decrease in the effect of angiotensin II results in normalisation of blood pressure and improved perfusion.
- Hydrochlorothiazide induces hypotension by causing sodium (salt) and water loss through urine. As sodium is lost, the amount of fluid in the blood vessels is also reduced and consequently, the pressure within the blood vessels drops.
Hence, these actions help ensure that the blood pressures remain within the normal limits and protect against the development of complications like stroke, heart failure, and renal impairment.
Atacand HCT, as with any other drug, is capable of having side effects, but may not be experienced by all. Some possible side effects include the following:
- Dizziness or light-headedness, particularly upon sudden changes of position
- Pain in the head
- Lethargy or tiredness
- Feeling nauseated or having an upset abdomen
- Edema, mainly in the lower limbs (ankles or legs)

Potassium-Sparing Diuretics and Supplements: Atacand HCT contains hydrochlorothiazide, a thiazide diuretic, which can cause low potassium levels (hypokalemia). Using it with potassium-sparing diuretics (e.g., spironolactone) or potassium supplements may increase the risk of hyperkalemia (high potassium levels), which can cause serious heart rhythm problems.
ACE Inhibitors and Angiotensin Receptor Blockers (ARBs): Taking Atacand HCT with ACE inhibitors (e.g., enalapril) or other ARBs (e.g., losartan) increases the risk of side effects like low blood pressure (hypotension), kidney problems, and elevated potassium levels. Close monitoring is essential when combining these medications.
Nonsteroidal Anti-Inflammatory Drugs (NSAIDs): NSAIDs (such as ibuprofen or naproxen) can reduce the blood pressure-lowering effect of Atacand HCT and increase the risk of kidney damage, particularly when used for extended periods. Patients should be monitored for kidney function and blood pressure.
Lithium: Atacand HCT can increase lithium levels in the blood, leading to lithium toxicity. Symptoms of toxicity include nausea, tremors, and confusion. If combined, lithium levels should be closely monitored and adjustments made as necessary.
Atacand HCT is usually initiated with an effective dose of 16 mg/12.5 mg once-daily. After assessing how well the patient took the medicine, the physician may adjust the dose to a larger one. Generally the maximal dose recommended is 32 mg/25 mg taken once a day.
If the dosage is altered it should be in accordance with the physician’s instructions. Changing the dose without permission is not advisable. Doses are adjusted to the most appropriate value by the physician depending on blood pressure levels and a patient’s condition.
