Each medication in this combination works in a different way to stop HIV from replicating:
- Efavirenz binds directly to the reverse transcriptase enzyme, causing a conformational change that prevents the enzyme from working. This halts the conversion of HIV RNA into DNA, which is a critical step in the virus’s replication process.
- Emtricitabine mimics the building blocks of DNA and gets incorporated into the viral DNA, causing premature termination of the chain. This stops the virus from replicating properly.
- Tenofovir also acts as a DNA chain terminator. It blocks the reverse transcriptase enzyme and, when incorporated into the viral DNA, stops replication.
Together, these drugs significantly reduce the viral load (the amount of HIV in the bloodstream), allowing the immune system to recover and reducing the risk of transmitting the virus to others.
Like all medications, Efavirenz, Emtricitabine, and Tenofovir can cause side effects. Some of the common and serious side effects include:
Common Side Effects:
- Headache
- Fatigue
- Nausea or vomiting
- Diarrhea
- Rash (mostly with Efavirenz)
- Dizziness or lightheadedness
Serious Side Effects:
- Liver problems: Signs include jaundice (yellowing of the skin or eyes), abdominal pain, and fatigue.
- Kidney problems: Tenofovir may cause kidney toxicity, so regular kidney function tests are recommended.
- Bone problems: Long-term use of Tenofovir can cause bone loss or fractures.
- Mental health issues: Efavirenz can cause or worsen depression, anxiety, or suicidal thoughts in some patients, especially in the first few weeks of treatment.
- Lactic acidosis: A rare but serious condition that can cause nausea, abdominal pain, and fatigue.
Severe allergic reactions: Though rare, some patients may experience severe allergic reactions like rash, swelling, or difficulty breathing.
- Antacids or Supplements: Tenofovir and Efavirenz may have their absorption affected by certain medications, like antacids or supplements containing calcium or magnesium.
- Other HIV Medications: Caution is needed when combining this regimen with other antiretroviral agents, such as protease inhibitors (PIs) or integrase inhibitors, as they may affect the drugs’ effectiveness or lead to resistance.
Hormonal Birth Control: Efavirenz may reduce the effectiveness of hormonal contraceptives, so an alternative or additional method of contraception should be used.
Efavirenz, Emtricitabine, and Tenofovir are prescription-only medications.
