- Administration: These medications are usually taken once a day in a fixed-dose combination tablet, with or without food.
- Timing: It’s recommended to take the medication at the same time every day to help with adherence to the treatment plan.
Missed Dose: If a dose is missed, take it as soon as you remember, unless it is almost time for the next dose. Do not double up on doses to make up for a missed one.
Each of the three drugs plays a different role in blocking HIV replication:
- Efavirenz (NNRTI): Efavirenz binds to the reverse transcriptase enzyme, causing a change in its shape that prevents HIV from converting its RNA into DNA. This prevents the virus from being integrated into the host cell’s genetic material.
- Lamivudine (NRTI): Lamivudine mimics one of the building blocks of DNA, and when the virus attempts to incorporate it into its DNA, it causes a premature termination of the viral DNA chain, preventing the virus from replicating.
- Tenofovir (NRTI): Tenofovir works similarly to Lamivudine by incorporating into the viral DNA and causing premature termination of the DNA chain, preventing viral replication.
Together, these drugs suppress HIV replication, help reduce viral load, and improve immune function by increasing the CD4 count, a measure of immune cell activity.
Like all medications, this combination therapy can cause side effects. These may include:
Common Side Effects:
- Headache
- Dizziness
- Fatigue
- Nausea or vomiting
- Diarrhea
- Rash
Serious Side Effects:
- Liver problems: Signs may include yellowing of the skin or eyes (jaundice), abdominal pain, or unusual fatigue.
- Kidney problems: Tenofovir has been associated with kidney toxicity in some cases, and regular kidney function monitoring may be necessary.
- Bone problems: Long-term use of Tenofovir may lead to bone loss or fractures.
- Mental health effects: Efavirenz can cause depression, anxiety, and suicidal thoughts, especially in the early stages of treatment.
Lactic acidosis: Though rare, this life-threatening condition can occur, and symptoms include nausea, abdominal pain, and fatigue.
- Liver Disease: Regular liver function tests are important for individuals with pre-existing liver conditions.
- Mental Health: Efavirenz can cause or worsen psychiatric issues, so people with a history of depression or mental health disorders should use caution.
- Pregnancy: Efavirenz is not recommended during the first trimester of pregnancy due to potential birth defects, but it may be used later in pregnancy if necessary.
- Kidney Function: Monitoring kidney function is important when using Tenofovir, particularly in individuals with pre-existing kidney disease.
Drug Resistance: Stopping treatment or taking it irregularly can lead to drug resistance. It is crucial to take the full prescribed regimen.
- Efavirenz: 600 mg once daily.
- Lamivudine: 150 mg twice daily.
- Tenofovir: 300 mg once daily.
These doses may be adjusted depending on individual needs, especially for patients with liver or kidney issues.
Efavirenz, Lamivudine, and Tenofovir are prescription-only medications. They should only be used under the supervision of a healthcare provider, who will monitor the patient’s condition, viral load, kidney and liver function, and potential side effects. Patients should not stop taking their medications without consulting their doctor.
