Fingolimod is taken by mouth as a capsule, usually once a day, with or without food. Here’s how to use it safely:
- First dose monitoring: The first dose is usually taken in a healthcare setting because Fingolimod can slow the heart rate. Your heart will be monitored for at least 6 hours after the first dose.
- Daily use: Take it at the same time each day to help you remember.
- Swallow whole: Do not open, crush, or chew the capsule.
Do not skip doses: If you miss a dose, especially in the first few weeks, call your doctor. Missing doses may require you to restart under medical supervision.
Fingolimod works by affecting the immune system. In MS, the immune system mistakenly attacks parts of the central nervous system. Fingolimod keeps certain white blood cells (called lymphocytes) from leaving the lymph nodes. By doing this, it prevents them from reaching the brain and spinal cord, where they can cause damage.
By reducing the number of immune cells that reach the nervous system, Fingolimod helps:
- Lower inflammation
- Reduce the number of MS relapses
- Slow down physical disability caused by MS
Like all medicines, Fingolimod can cause side effects. Some are mild, while others can be serious and need medical attention.
Common Side Effects:
- Headache
- Flu-like symptoms
- Diarrhea
- Back pain
- Cough
Serious Side Effects:
- Slow heart rate (especially after the first dose)
- Increased risk of infections
- Liver problems (seen as yellowing of the skin or eyes, dark urine)
- Vision problems due to swelling in the back of the eye (macular edema)
- High blood pressure
- Breathing problems
If you notice signs of infection (fever, chills), liver trouble (nausea, yellowing of skin), or vision changes, contact your doctor immediately.
Before starting Fingolimod, your doctor will check if it is safe for you. Some important things to keep in mind:
- Heart problems: Fingolimod can slow your heart rate, especially after the first dose. You may need ECG monitoring.
- Infections: Fingolimod lowers immune defense, so you’re more prone to infections. Avoid contact with sick people.
- Eye health: Before starting Fingolimod, get an eye exam. If you have diabetes or a history of eye problems, you may be at higher risk of macular edema.
- Liver function: Your doctor will check your liver enzymes before and during treatment.
- Vaccines: Avoid live vaccines while taking Fingolimod and for 2 months after stopping.
Pregnancy: Fingolimod may harm an unborn baby. Women should use effective birth control during treatment and for 2 months after stopping the medicine.
Fingolimod can interact with other medicines. Some important interactions include:
- Heart medications: Fingolimod can slow the heart rate, so combining it with drugs that also affect the heart (like beta-blockers or calcium channel blockers) can increase risk.
- Vaccines: You should avoid live vaccines during and for 2 months after Fingolimod treatment because your immune response may be weakened.
- Other immune system drugs: Taking Fingolimod with other immunosuppressants can increase the risk of infections.
- Ketoconazole and other CYP3A4 inhibitors: These can increase the levels of Fingolimod in the blood and may lead to side effects.
Always tell your doctor about all medicines and supplements you’re using before starting Fingolimod.
- Adults: The standard dose is 0.5 mg once daily, with no dose adjustment needed for age or weight.
- Children (10 years and older):
- Children weighing ≥40 kg: 0.5 mg once daily
- Children weighing <40 kg: 0.25 mg once daily
Important: If you stop taking Fingolimod for more than:
- 1 day in the first 2 weeks
- 7 days in weeks 3 and 4
- 14 days after 1 month of treatment
…you may need to restart under medical supervision due to heart risks.
Fingolimod is a prescription-only medicine. It should be prescribed by a neurologist or specialist who has experience in treating multiple sclerosis.
Before prescribing Fingolimod, your doctor will:
- Do a baseline ECG
- Check liver function tests
- Perform a complete blood count (CBC)
- Screen for infections, including chickenpox (varicella)
- Check your eye health
You will also be monitored regularly while using Fingolimod to ensure your safety.
