Fintepla comes as a liquid solution that you take by mouth using a special measuring syringe provided with the medicine.
Instructions for use:
- Take Fintepla once or twice daily, as directed by your doctor.
- Use the syringe provided to measure the exact dose.
- It can be taken with or without food.
- Do not drink alcohol or use illicit drugs while on Fintepla.
- If you miss a dose, take it as soon as possible. If it’s close to the next dose, skip the missed one. Do not double the dose.
Important: Patients on Fintepla must be enrolled in the Fintepla REMS program, a safety program that monitors for heart problems (explained below).
Fintepla works in the brain by increasing the activity of serotonin, a chemical that helps regulate mood and brain activity. It is thought that by affecting serotonin, Fintepla can reduce the electrical signals in the brain that cause seizures.
While the exact way it works is still being studied, it has been shown to lower the frequency of seizures in children and adults with Dravet and Lennox-Gastaut syndromes.
Fintepla also has other actions in the nervous system that may help control excessive brain activity, making it useful for certain epilepsy syndromes that do not respond to traditional treatments.
Like all medicines, Fintepla may cause side effects. Some are mild, but others may be serious and require close monitoring.
Common Side Effects:
- Sleepiness or drowsiness
- Loss of appetite
- Weight loss
- Diarrhea
- Fatigue
- High blood pressure
Serious Side Effects:
- Heart valve problems
- Pulmonary arterial hypertension (PAH) — high blood pressure in the lungs
- Serotonin syndrome — can occur if combined with other serotonin-increasing drugs
- Behavioral changes — such as aggression or mood swings
Because of the risk of heart-related side effects, regular heart checks (echocardiograms) are required during treatment.
Fintepla comes with important safety warnings. Before starting, patients must go through a screening process and follow specific guidelines.
Important Warnings:
- Heart valve disease and PAH: Fintepla can cause serious heart conditions. Regular echocardiograms (heart ultrasounds) are required before and during treatment.
- Serotonin syndrome: Combining with certain antidepressants or migraine medications can raise serotonin to dangerous levels.
- Decreased appetite/weight loss: Common in children. Growth and weight should be monitored.
Who should not take Fintepla:
- Patients with heart valve problems or a history of pulmonary hypertension
- Anyone allergic to fenfluramine
Patients who are pregnant or breastfeeding should speak to their doctor, as the safety of Fintepla in pregnancy is not well known.
Fintepla can interact with other medicines, especially:
- Stiripentol: Changes the way Fintepla is metabolized.
- Other seizure medications: May affect Fintepla’s effectiveness or side effects.
- Serotonin-related drugs (like antidepressants): Can increase the risk of serotonin syndrome, a serious condition caused by too much serotonin in the body.
Always inform your doctor of all medications and supplements you’re taking before starting Fintepla.
Your doctor will decide the best dose based on your weight, age, and other medications you are taking.
- Start with a low dose and slowly increase to reduce the risk of side effects.
- Maximum dose depends on:
- If you are also taking stiripentol
- Your weight
- Your response to treatment
Regular follow-ups are needed to check:
- Heart function
- Blood pressure
- Weight and appetite
Never increase or decrease your dose without your doctor’s advice.
Fintepla is a controlled substance and can only be prescribed by doctors enrolled in the Fintepla REMS program. This is a special program in the U.S. and other countries to ensure patient safety.
To receive Fintepla:
- You must register in the REMS program.
- You will need regular heart tests (echocardiograms).
- Your pharmacy must be authorized to dispense Fintepla.
It cannot be bought over the counter or online without a prescription and REMS enrollment.
